Do you meet the criteria for funded treatment?
Before you start your journey as a funded subfertility patient, you will need to know whether you meet the criteria set by your local CCG (Clinical Commissioning Group).

NHS funding can cover the costs of in vitro fertilisation (IVF) or intra-cytoplasmic sperm injection. The number of cycles (rounds of treatment) available varies dependant upon your CCG. For more information see also the NHS website – www.nhs.uk

Your General Practitioner will need to complete the sub fertility clinic referral form including answering questions regarding your suitability in meeting the criteria for funded treatment. He/she will also need to forward results of specific investigations carried out prior to your referral into the clinic.
